Patients wanting appointments can start the process by Downloading the Self-Referral Form, fill it up and submit by Fax or Post. Physicians or Hospitals who wish to refer a patient can Download the Physician Referral Form, fill it up and submit it by Fax or Post. All e-mail Referral Forms will receive a response after 2 working days. Referrals may also be made by Phone or Fax.

When the Medical Coordination Office contacts you the following information will be required:
    1. Patient name, telephone number, Iqama number 
    2. Referring patient's name, office address, Telephone and Fax numbers.
    3. Referral letter
    4. Medical report
    5. Lab report
    6. Diagnosis
    7. Date of diagnosis 
    8. How the diagnosis was made (physical exam, biopsy, other)
    9. What treatment, if any, has taken place over what time period the treatment has occurred.
As assessment will be performed prior to confirming an appointment. Specific medical and pathology reports may also be requested for review before your final visit.
The Patient Referral Process in the Outpatient Department (Eligibility process)
   * Please ensure that all the relevant documents are available with you. These include 1. Iqama/ID, 2. Referral Letter, 3. Medical reports 
      including lab reports, sample tissue, etc. 
   * Visit the RECEPTION DESK (OPD) with all the documents.
   * You will be sent either to Patient Affairs Department, Oncology Center or Emergency Room.
   * You will then be referred to the Medical Coordination Office, to assess your acceptability For treatment in our hospital.
   * If all your documents are not in place, you will have to re-visit us after all the documents are in place.
OR 
   * If your documentation is complete, you will be processed further.
   * You may not be accepted for treatment, if we do not have the Specialty Service available. The referring physician/hospital will be informed
     of this decision.
OR
   * You may be Accepted for Treatment when the Specialty Service is available.
Once accepted, a temporary Medical File is Opened and You will be given an appointment to the Specialty OPD. The consultant will then decide on a treatment plan for you either on a n Outpatient or Inpatient basis.
